SAN FRANCISCO, CA -- November 1, 2007 -- Magnetic resonance arteriography/venography (MR-A/V) is not accurate enough to structure management of impalpable testes, according to a study presented here at the American Academy of Pediatrics (AAP) National Conference and Exhibition.

The prospective study was presented by investigator Naresh Desireddi, MD, Department of Urology, Children's Memorial Hospital, Chicago, Illinois, United States.

Because testis parenchyma and vascular axis can be imaged by MR-A/V with gadolinium intravenous injection, the technique has been applied to aid the clinical management of paediatric impalpable testes. The test is reported to be able to help discern with more than 95% accuracy if an impalpable testis is intra-abdominal or a testicular nubbin. However, relying on imaging studies may lead to misdiagnosis and thereby conclude that surgery is not needed.

The study involved 26 boys (including 29 nonpalpable testes) who were identified as having an impalpable testis during office examination and who were evaluated by MR alone or MR-A/V (depending upon scheduling availability). Fourteen boys (14 testes) underwent MR-A/V. All cases underwent surgery. A single radiologist, blinded to the clinical findings, reviewed the imaging tests for testicular presence and location.

Findings based on imaging studies were found that have an overall accuracy rate of 60%, with diagnosis of abdominal testis being 80% accurate, canalicular 67% accurate, and nubbin 40%.

The authors of the study concluded that MR-A/V findings are concordant with surgery little more than half the time and are thus too unreliable to structure management of paediatric impalpable testes.

They recommended that cases of impalpable testes should undergo surgical management without MR imaging studies.
