Title: Aggressive Surgery May Offer Good Survival Rates in Recurrent Gastric Cancer: Presented at ESSO

THE HAGUE, Netherlands -- September 11, 2008 -- Aggressive surgery may result in good survival rates for patients with a locoregional recurrence of gastric cancer, researchers reported here at the 14th Congress of the European Society of Surgical Oncology (ESSO).

Vladimir Khomiakov, MD, Department of Thoraco-Abdominal Oncology, Moscow Herzen Oncology Research Institute, Moscow, Russia, presented the findings on September 10.

The study evaluated 984 patients with stage I to III gastric cancer who had undergone potentially curative resection surgery.

Following the initial surgery, disease relapse occurred in 48% of patients. Seven percent experienced a locoregional recurrence, 10% had both local and systemic recurrence, and 31% of patients had distant metastases only.

A total of 220 patients with a locoregional recurrence were further evaluated according to whether or not they underwent subsequent surgery for their recurrent gastric cancer. The recurrence appeared in the gastric stump in 47 patients (21.4%), in the anastomosis in 74 patients (33.6%), and in the perigastric zone in 99 patients (45%).

According to Dr. Khomiakov, 104 patients (47.3%) underwent surgery for the recurrent tumour. Forty patients received the most aggressive type of surgery for their condition -- R0 (curative) surgery. Palliative resection (R1/R2) was done in a further 3 patients with distant metastases, and exploratory or symptomatic surgery (such as gastric bypass or enterostomy) was done in 61 patients.

The findings showed that the 5-year survival rate in the patients who had R0 surgery was 40%. Their 10-year survival rate was 16%, and the 5-year Kaplan-Meier estimate of survival was 38.7%. These survival rates were higher than in the other 2 types of surgery patients.

In addition, the investigators found that the median survival in patients who underwent exploratory or symptomatic surgery was 5.6 +- 1.8 months.

Dr. Khomiakov concluded that R0 resection resulted in the best survival rate for patients with a locoregional recurrence of gastric cancer.
