CHICAGO -- September 26, 2008 -- Initial clinical stage is an important predictor of outcomes in patients surgically treated for squamous cell carcinoma of the hypopharynx, according to a study presented here at the American Academy of Otolaryngology - Head and Neck Surgery Foundation (AAO-HNSF) 2008 Annual Meeting & OTO EXPO.

Helma Maria Chedid, MD, Department of Head and Neck Surgery and Otolaryngology - Hospital Heliopolis, Sao Paulo, Brazil, and colleagues reported that patients with advanced clinical stage at the initial presentation had lower disease-free survival than those presenting with earlier-stage disease.

"In our study, 168 patients had advanced clinical stage (III and IV) disease with 5-year disease-free survival of 75% for patients with stage III disease versus 28% in patients with stage IV disease," the researchers said in their poster, presented on September 21.

Dr. Chedid and colleagues retrospectively evaluated the charts in 174 patients with squamous cell carcinoma of the hypopharynx who underwent surgical treatment between January 1999 and December 2005.

Most of the patients presented with advanced disease; 46 (26.4%) presented with clinical stage III disease and 122 (70.1%) with clinical stage IV disease, 2 (1.2%) patients presented with clinical stage I disease, and 4 (2.3%) with clinical stage II disease.

All patients underwent either pharyngolaryngectomies or total laryngectomies, with 222 neck dissections performed (48 bilateral). Postoperative radiotherapy at a mean dose of 52.8 Gy was given to 106 patients.

At a mean follow-up of 31 months after treatment, histological examination showed 57 patients with pathological T4 disease, 100 with T3 disease, 15 with T2 disease, and 2 with T2 disease. Most patients had positive lymph nodes (n = 149).

The researchers concluded that most patients presented with advanced clinical stage disease (96.5%) with a disease-free survival of 35% and overall survival of 28%.

According to Dr. Chedid, these survival rates are similar to other rates reported in the literature.
