GENEVA -- May 21, 2008 -- Weight-loss therapy with the anti-obesity drug orlistat is an effective first option for obese adolescents with menstrual disorders, researchers reported here at the 16th European Congress on Obesity (ECO).

Russian researchers set out to investigate the efficacy of orlistat on body weight, metabolism, and the restoration of ovulation in obese adolescents with menstrual disorders, led by Irina Kuznetsova, MD, Department of Obstetrics and Gynaecology, Russian Medical Academy of Postgraduate Education, Moscow, Russia.

The team examined 32 adolescents between the ages of 14 and 18 who had a mean body mass index (BMI) of 33.4. Twenty-four of the adolescents had oligomenorrhoea (infrequent or light menstruation), 5 had amenorrhoea (no menstruation), and another 3 had dysfunctional uterine bleeding.

The patients were measured before treatment for body composition, glucose, insulin, and lipids. They also received a hormonal profile.

Treatment consisted of orlistat 360 mg/day for 6 months along with a hypocaloric diet and exercise.

Five of the adolescents dropped out within the first month due to side effects attributable to orlistat.

At the completion of treatment, menstrual function had improved in the vast majority of the adolescents (81.5%). Spontaneous ovulation was observed in 37% of these patients.

The 27 adolescents who remained in the study showed an average weight loss of 8.13 kg, an average BMI reduction of 8.69%, and significantly improved insulin sensitivity (P = .002).

These subjects also showed improvements in total cholesterol, low- and high-density lipoprotein cholesterol, and triglycerides. Their hormonal profiles demonstrated significantly decreased concentrations of testosterone, androstenedione, and leptin.

"Weight-loss therapy with orlistat is effective, and should be considered as a first option for obese adolescents with menstrual abnormality," said Dr. Kuznetsova. "However, gastrointestinal side effects limit its usefulness."
