GENEVA -- May 21, 2008 -- Patients with the highest presurgical body mass indices (BMI) and who achieved the most weight loss before surgery achieved the most postoperative weight loss after insertion of an adjustable gastric band in a study presented here at the 16th European Congress on Obesity (ECO).

The researchers set out to learn what factors might predict total postoperative weight loss following laparoscopic gastric band insertions (LGBI). They analysed patients' weight at initial assessment, age, gender, and preoperative weight loss, as well as the type of gastric band used.

Mubashir Mulla, Medical Student, Trent Regional Service for Bariatric Surgery, Department of General Surgery, Derby Hospitals, Derby, United Kingdom, and colleagues performed a retrospective analysis on patients who underwent LGBI over 3 years at a regional centre for bariatric surgery in the United Kingdom.

The study, presented on May 17, was conducted with Mike Larvin, MD, Professor of Surgery, University of Nottingham Medical School at Derby, Derby, United Kingdom, and other colleagues.

The 197 women and 49 men were enrolled if they had a BMI of 40 kg/m² or greater or a BMI of 35 kg/m² or greater if with an associated comorbidity.

Before surgery, patients received professional dietary counselling and were instructed to follow a calorie-restricted diet for 2 weeks before surgery.

All patients were inserted with an adjustable gastric band device.

A patient's initial weight correlated significantly with the amount of weight lost prior to LGBI (Spearman's P < .0001). The 75 patients with BMI of 50 kg/m² or greater lost an average of 6.4 kg before the surgery. By comparison, the 109 patients in the lower BMI range lost an average of 2.1 kg.

In addition, presurgical weight loss correlated with postsurgical weight loss (Spearman's P = .016,).

Postoperative weight loss was not correlated with age, gender, or brand of the device inserted.

Patients with high baseline BMI and those with the greatest weight loss prior to undergoing LGBI appear to achieve the greatest weight loss after LGBI, the study's authors concluded. "It appears that preoperative diet predicts post-LGBI compliance and thus weight loss."
