BOSTON, MA -- February 6, 2008 -- Once-daily atazanavir boosted with ritonavir (ATV/r) is as effective as twice-daily lopinavir boosted with ritonavir (LPV/r) in treatment-naive patients with HIV who were also taking fixed-dose tenofovir/emtricitabine, according to study results presented here at the 15th Conference on Retroviruses and Opportunistic Infections (CROI).

Lead author Jean-Michel Molina, MD, PhD, Head, Department of Infectious Diseases, St Louis Hospital, and Professor, Infectious Diseases, University of Paris - Diderot Paris 7, Paris, France, presented the study on February 4.

Dr. Molina noted that ATV/r demonstrated similar efficacy, a lower incidence of gastrointestinal-related adverse events, and a significantly better lipid profile compared with LPV/r.

The interim data from the ongoing, 96-week CASTLE study examined 883 treatment-naive patients with HIV who were randomized to receive either ATV/r (n = 440) or LPV/r (n = 443). At week 48, mean CD4 increases from baseline were 203 and 219 cells/mm3 for ATV/r and LPV/r, respectively.

Fewer patients on ATV/r (2%) than on LPV/r (7%) initiated lipid-lowering therapy. The rates of patients with ratios greater than 5 for triglycerides to high-density lipoprotein at week 48 were 12% and 20% in the ATV/r and LPV/r groups, respectively.

Patients on ATV/r had a lower incidence of grade 2 to 4 treatment-related adverse events than those on LPV/r (diarrhea, 2% vs 11%; nausea, 4% vs 8%).

Both regimens were generally well tolerated, with low rates of treatment discontinuation.

Dr. Molina concluded that ATV/r is an appropriate therapeutic option for treatment-naive patients, and is highly effective in achieving virus undetectability in patients with advanced disease.
