Rabeprazole, in combination with two antibiotics, can effectively eradicate [Helicobacter Pylori infection, say Spanish researchers. Eradication rates with the combination are similar to those achieved by omeprazole and lansoprazole when co-prescribed with antibiotics. This systematic review of reported studies on the efficacy of rabeprazole-based therapies and a meta-analysis comparing it to other proton pump inhibitors plus antibiotics was conducted by J.P. Gisbert and researchers at the Hospital Universitario de la Princesa, Madrid, and Hospital de Sabadell, Institut Universitary Parc Tauli, Barcelona, Spain. Their meta-analysis of randomised trials included those comparing rabeprazole and other proton pump inhibitors with antibiotics. Electronic and manual bibliographic searches and calculations were made of the percentage (weighted mean) of successful eradication. Odds ratios (OR) of the individual studies were combined for the meta-analysis. Eradication rates for the 14-day rabeprazole-amoxicillin combination were 73% and for rabeprazole-amoxicillin-clarithromycin 44% at 3 days, 72% at 5 days, 78% at 7 days and 75% at 10 days. For low-dose rabeprazole, 20 mg/day plus amoxicillin and clarithromycin, the eradication rate over 7 days was 81% and for high-dose rabeprazole, 40 mg/day, with amoxicillin and clarithromycin, 75%. For 7 days with the rabeprazole-clarithromycin-nitroimidazole combination, it was 85%. The meta-analysis included 12 comparative studies. With rabeprazole plus antibiotics the eradication rate was 79% and with other proton pump inhibitors it was 77% (OR = 1.15; 95% confidence interval, 0.93-1.42). No difference was seen in sub-analysis comparison of rabeprazole at low doses,10 mg twice daily, with other proton pump inhibitors at standard doses, omeprazole 20 mg twice daily or lansoprazole 30 mg twice daily.
