Short duration cataract surgery may be associated with phototoxic lesions. Previous studies have suggested that prolonged cataract surgery, especially procedures longer than 100 minutes, seems to be associated with an increased risk of microscope-induced retinal phototoxicity. To further examine this relationship, researchers from Kaplan Medical Center, Rehovot, and other centres in Israel, retrospectively enrolled 34 patients who developed phototoxic retinal lesions following either phacoemulsification or extracapsular cataract extraction and intraocular lens implantation. The authors stratified patients into two groups. The first group included 14 patients whose operating time was 30 minutes or less. The second group comprised 20 patients in whom the operation lasted more than 30 minutes. Mean operating times were 23.1 and 60.8 minutes respectively, with ranges of 11 to 30 minutes and 34 to 123 minutes respectively. Patients in whom the operation lasted 30 minutes or less were more likely to undergo phacoemulsification than those whom underwent longer procedures. On the other hand, extracapsular cataract extraction was more common in those in whom the procedure was prolonged. Moreover, more patients in the short duration group showed diabetic retinopathy. Eighty-six percent of eyes operated on for 30 minutes or less showed a final refraction of ± 1 D. This compared to 60 percent of those in whom the operation lasted more than 30 minutes. Moreover, the closer final refraction came to emmetropia, the shorter surgical duration. The authors highlighted the approximate association between emmetropia and diabetic retinopathy as the most relevant factor revealed by the study. The authors concluded that even short duration cataract surgery could be associated with phototoxic lesions.
