        Summary points:A 59-year-old Caucasian female, with a history of hip replacement, presents with rest pain, numbness, and cramps in the operated limb. Vascular reconstruction is described; images provided. The authors discuss a rare presentation of an occlusion of the common femoral artery by cement leakage under the transverse ligament during cemented total hip replacement, and stress the importance of correct technique of pressurisation to avoid this type of complication.
