      Summary points:A 23-year-old presents with respiratory distress, abdominal pain, and severe left-lower-extremity pain, swelling, and purplish discoloration, with the left side larger than the right. This case report describes the clinical presentation, findings from hypercoagulable workup and venogram, treatment, and outcome, and includes several images. The authors review the literature on duplication of the inferior vena cava and describe its pathophysiology, diagnostic methods, and treatment.
