      Summary points:A 38-year-old with underlying beta-thalassemia/haemoglobin E disease presents with sudden shortness of breath and right-sided chest pain. This case describes the physical examination and laboratory findings, as well as from chest radiograph, thoracentesis, and computed tomography, the treatment and patient outcome. The authors also discuss the incidence, pathogenesis and common presentation of extramedullary haematopoiesis.
