      Summary points:The authors report a case of haemolysis, elevated liver enzyme levels, and low platelet count (HELLP) syndrome in a 26-year-old, pregnant female with a 12-year history of systematic lupus erythematosus and antiphospholipid syndrome (APS). Imaging confirmed intrauterine foetal death in the 17th week of pregnancy and multiple liver infarctions. Laboratory data, urinalysis, ultrasound examination, histopathological examination, abdominal computed tomography, and treatment are described. The authors highlight the characteristic features of HELLP syndrome associated with APS, and suggest that at-risk patients be followed up carefully.
