      Summary points:A 20-year-old former occasional smoker presents with fever, progressive dyspnoea, and pulmonary infiltrates despite initial antibiotics. This case report describes the history, laboratory and radiological findings, treatment, and outcome, and includes some images. The authors discuss the possible causes of idiopathic acute eosinophilic pneumonia, differential diagnosis, and treatment.
