      Summary points:A 64-year-old with 3-month history of refractory anaemia with multilineage dysplasia and ringed sideroblasts presents with low-grade fever, fatigue, and insomnia. This case report describes the findings from laboratory and radiological tests, bone marrow aspirate analysis, the diagnosis, and treatment outcome. The authors describe miliary tuberculosis, symptoms, diagnosis, and treatment.
