The authors present a case of a 29-year-old woman diagnosed with type 1 diabetes who received insulin aspart and NPH insulin (Novolin). On day 22, she experienced leg oedema and right abdominal pain. Serum hepatobiliary enzyme levels were markedly elevated and computed tomography revealed gallbladder oedema. After an injection of human regular insulin and NPH insulin (Humacart), the elevated liver enzyme levels were no longer observed. The authors disucss an extremely rare case of a delayed-type protamine allergy caused by Novolin resulting in gallbladder oedema.
