news Gallbladder Edema in Type 1 Diabetic Patient due to Delayed-type Insulin Allergy

Presented by:Internal Medicine / Fumiko Kawasaki, Shinji Kamei, Fuminori Tatsumi, Sumiko Hamamoto, Masashi Shimoda, Kazuhito Tawaramoto, Makoto Shigeto, Yukiko Kanda, Mitsuru Hashiramoto, Michihiro Matsuki and Kohei Kaku
Summary points:The authors present a case of a 29-year-old woman diagnosed with type 1 diabetes who received insulin aspart and NPH insulin (Novolin). On day 22, she experienced leg oedema and right abdominal pain. Serum hepatobiliary enzyme levels were markedly elevated and computed tomography revealed gallbladder oedema. After an injection of human regular insulin and NPH insulin (Humacart), the elevated liver enzyme levels were no longer observed. The authors disucss an extremely rare case of a delayed-type protamine allergy caused by Novolin resulting in gallbladder oedema.
