        Summary points:A 14-year-old presents with worsening paraplegia, ataxia, fever, and fatigue that started a few months earlier and has been getting worse in the last 3 weeks. This case report describes the laboratory and magnetic resonance imaging findings, response to initial treatment, follow-up cultures, treatment, and final outcome, and includes some images. The authors discuss the pathophysiology of vitamin B12 deficiency, its clinical features, and treatment, and review the literature.
