      Summary points:A 45-year-old male who undergoes orthotopic heart transplantation for dilated cardiomyopathy suddenly develops profound cardiogenic shock 8 days post-transplantation. This report describes the transplantation procedure, immunosuppresive therapy, postoperative graft heart function, echocardiography, treatment response to cardiogenic shock, and outcome. The authors discuss monitoring for, diagnosis of, and effective treatment options for humoral rejection.
