        Summary points:A 52-year-old male noted distal dominant slowly progressive muscle weakness at age 36 years. Physical examination, laboratory tests, computed tomography, and muscle biopsy are described; images provided. The authors discuss a unique form of distal myopathy characterised by the presence of multi-minicores with preferential involvement of type 1 fibers.
