        Summary points:A 31-year-old healthy Japanese man with a 1-month history of low-grade fever and epigastric pain is referred for detailed examinations and treatment of liver dysfunction and gastric lesions. Medical history, physical examination, laboratory data, endoscopy, contrast enhancement computed tomography, immunohistochemical examination, serological tests, and colonoscopy are described. The authors discuss the case of an immunocompetent adult with acute gastric mucosal lesion that derived from cytomegalovirus infection.
