| Summary points: | A 50-year-old woman presents with progressive onset bilateral shoulder pain. She had no previous injury to or problems with her shoulders. Nine months previously, she noted that decreased range of motion of the shoulders became more persistent and accentuated. Physical examination, laboratory data, plain radiographs, computed tomography, spinal magnetic resonance imaging, and synovial tissue studies from biopsies of the shoulder are described. The authors describe their diagnosis of bilateral idiopathic destructive arthritis and discuss its remarkable characteristics, radiographic features, and clinical presentation. |
