      Summary points:A 64-year-old male, with a history of rheumatoid arthritis (RA) and acute myeloid leukaemia with multilineage dysplasia (AML-MLD), presents with high fever and dyspnea. Laboratory tests, chest x-ray, computed tomography, and autopsy results are described; images provided. The authors discuss an extremely rare case of co-existing AML-MLD and Epstein-Barr virus-associated T-cell lymphoproliferative disorder in a patient with RA.
