        Summary points:A 34-year-old with migraine, hypertension and hypothyroidism presents with acute bilateral severe visual loss after increasing her dose of topiramate 1 week after initiating therapy. This case report details the history, clinical presentation, and B-scan ultrasound findings, treatment, and outcome, as well as providing some images. The authors review the literature and discuss the implications for treatment of patients presenting with headache.
