        Summary points:A 24-year-old man presents with a 1-week history of on-and-off fever and diffuse abdominal pain and a 2-day history of constipation, vomiting, and abdominal distention. Physical examination, laboratory data, abdominal ultrasonography, and emergency laparotomy are described. The authors discuss the very rare, but very dangerous complication of intestinal strangulation caused by appendicitis.
