Summary points:
A 6-year-old with history of asthma presents with a sudden cutaneous rush and haemodynamic instability with severe hypotension 60 seconds after heparin administration while undergoing surgical correction of a ventricular septal defect. This case report details the anaesthetic procedure, the response to initial management of shock, and the final outcome. The authors discuss the pathophysiological mechanisms of anaphylactic and anaphylactoid reactions during anaesthesia and the role of vasopressin for treatment of vasodilatory shock.
